Role Description and Responsibilities
The Manager of Pharmacy is a registered pharmacist responsible for providing clinical leadership and strategy to support medication management for the organization. The Manager of Pharmacy will be responsible for driving medication adherence performance, disease management, and medication optimization.
The Manager of Pharmacy will directly manage a team of 12 Pharm Techs and serve as cohort leader for pharmacists in the organization.
This leader is proficient in project management and proactively uses data and analysis to track, benchmark, and communicate the outcomes of medication efforts to ensure operational, clinical quality, and organizational priorities are achieved.
The Manager of Pharmacy will report to the VP of Population Health.
Core Responsibilities:
Elevate the pharmacy program at Oak Street Health through meaningful contributions to overall health outcomes
Deliver evidence-based, patient-centered medication recommendations
Drive medication adherence strategy and achieve 4.5+ on Part D Stars
Optimize medication reconciliation process
Improve the management of A1c for patients living with diabetes, and contribute to broader disease management program improvement
Recognize the important role of behavior and lifestyle interventions alongside medication interventions in managing chronic disease, and have a track record of successful care collaborations with clinicians to support behavior change as part of holistic approach to chronic disease management
Support the implementation of clinical guidelines (e.g., hypertension, diabetes, heart failure)
Optimize tools and data around medication to equip care teams to better support patients with a well-optimized medication regimen
Directly manage and optimize the outcomes from Pharm Tech interventions
Define the structure and responsibilities for the regional Pharmacist role
Create interventions for Part D cost strategy

What are we looking for?
PharmD licensure required
Other advanced degree (MBA, MHA, MPH, Master of Science) preferred
Familiarity with CMS Stars and Medication Adherence strategy
Strong analytical skills
Excellent verbal and written communication skills
Demonstrated strategic and analytical problem-solving capabilities
Experience in clinical leadership roles, leading and coaching pharmacists and pharmacy techs
Ability to effectively and succinctly synthesize large amounts of information into a format that is easy for others to understand
Ability to function in an entrepreneurial environment and manage in the midst of ambiguity
Excited by the work of creating and implementing new processes in order to execute the Oak Street vision
Ability to collaborate effectively with executive staff, administration, providers, payer partners and managed care operations leaders
Passionate about their work and motivated to be a part of a fast growing organization
Self-aware and confident in their leadership skills
Strong belief in the value based care model and a passion to 'rebuild healthcare as it should be'
Proficiency in Excel required
