Role Description:

The Director role on the Population Health team will be a key member of the Field Services team, and will primarily serve as the lead for the Population Health Director (PHD) cohort that includes 15 PHDs and is expected to grow to +25 across the next 2-years. This role is responsible for onboarding, as well as ongoing training needs and general support for all PHDs. In addition to these responsibilities, the Director will provide temporary PHD support to new regions and when there is a gap in PHD coverage. This role will also serve as a key liaison between the PHD cohort and leadership across the organization.

The Director, should have previous experience as a PHD, and be a subject matter expert in all functions of the role. Key outputs of this role will be quick mastery of the role for all new PHDs, improved PHD coverage for all regions, and organization-wide improvements in key Population Health programs, including Clinical Documentation, Quality, and Medical Costs.

 

Core Responsibilities:

  • Partner with central and regional leadership to support PHD hiring, onboarding, ongoing training and evaluation

  • Provide ongoing feedback, mentorship and coaching to individual PHDs to support the rapid progression to mastery in their role

  • Convene key stakeholders to regular and ad hoc PHD cohort meetings, for organizational updates, trainings and collaboration 

  • Support working groups across the organization that impact key initiatives of the PHD cohort

  • Provide temporary support to regions without a PHD, due to transitions or extended leave

  • Support new regions in implementing Oak Street Health's care model, focused on critical Population Health components -- including, but not limited to, clinical documentation workflows and best practices, CIS program implementation, Quality program training, and VIP/Weekly Planning meeting training 

  • Facilitate dissemination of best practices and development of new tools to support standardization of role and more effective outputs

  • Partner with RVPs, SMDs and other leaders to identify areas of improvement and resolve knowledge gaps of any PHD

  • Other duties, as assigned

 

What are we looking for?

  • 1+ year experience as a regional Population Health Director in good standing

  • Expert in Oak Street Health business model, and key population health drivers of value to the organization

  • Highly organized and detail oriented, ability to proactively identify challenges and plan accordingly

  • Strong communication skills and high emotional intelligence – ability to connect and work effectively with diverse stakeholders

  • Dedicated to advancing the PHD program effectiveness and impact, and supporting the ongoing development of future leaders of Oak Street Health

  • U.S. Work Authorization
