Core Responsibilities:

  • Attendance at monthly care report registry reviews for Providers
  • Partnership with Population Health Director on general review support; Medical Records reviews
  • Access EMRs to identify potential suspects based on test results, etc.
  • Review EMRs to find support for suspected HCCs
  • Identify Medical Records requests from Blue Button; need for supporting documentation
  • Support for clinical peer review activities in support of the medical staff. Identify and evaluate patient care issues using applied knowledge, clinical experience, and analytical skills
  • Identify and analyze opportunities for improving patient quality outcomes and cost of care
  • Analyze chronic condition prevalence based on patient demographics and identify opportunities for interventions
  • Structure initiatives to drive improvement in quality and risk adjustment documentation results
  • Monitor clinical documentation efforts
  • Special Projects and Analysis, as assigned
  • Develop predictive analytics insights
  • Other duties, as assigned


What are we looking for?

  • Ease and expertise working with multiple technical systems, including Greenway, Canopy, and Tableau
  • Ability to collaborate across multiple teams and engage in multiple perspectives to work toward a common goal
  • Proven experience and ability to build relationships with Providers
  • Proficiency in tracking down and leveraging external resources, such as hospital records, Blue Button, and payer data, to support retrospective and prospective accurate documentation
  • Ability to succeed in a fast-paced, ever-changing environment
  • Skill in data analytics
  • Self-starter with a high degree of drive, initiative, and follow through
  • Ability to develop, plan, and implement short and long term goals
  • Exceptional interpersonal, written and verbal communication skills
  • Commitment to company vision
  • Access to reliable transportation for travel between regional center locations
  • Experience in accurate and specific documentation
  • U.S. work authorization
