Oak Street Health's impact on the communities we serve is large and growing. This Medical Director role will put you at the leading edge of the value-based care movement by being a part of our Central Care Model Development team.
The Medical Director for Population Health will be responsible for co-leading, with Directors and Vice Presidents from the Population Health and Central Services Departments, a range of clinical and strategic initiatives. The Population Health program is led by Oak Street's Chief Population Health and Chief Clinical Officers and focuses on the design and implementation of the Oak Street Care Model. The Oak Street Product team which designs, builds and implements software tools to support care team workflows and meet their information needs is a part of the Population Health team and therefore technology work will be part of the scope of the team and position. Although the vast majority of Oak Street's care delivery is organized through its Divisions and Regions, there are also centralized clinical efforts which most often reside within the Population Health or Centralized Services Teams. Core Responsibilities:

Depending on the program design, the Medical Director may be expected to provide direct clinical supervision to RN's, NP's, physicians, and/or other clinical or support staff involved in delivering care through these programs.
  • Liaison to other clinical and operational leaders: Patients receiving care through these clinical programs will, in most cases, also be receiving primary care through Oak Street's network of primary care centers. An Insider's view of Oak Street Health

What’s the vibe like in the office?

Everyone here truly believes in what we’re building which creates a collaborative and focused environment where we’re always willing to lend a hand. We love to work hard and also have fun every step of the way. There’s never a shortage of smiles and laughter during our team standups, lunches, happy hours, and company wide huddles.

Kelsey

Product Manager

What's the biggest problem your team is solving?

Traditional healthcare systems tend to leave care teams to figure things out on their own. Here, we apply today’s standards in UX and machine-learning to change that behavior for the better. Gathering direct feedback from our teams, we identify what’s most important to them and help them make clinically sound decisions for our patients’ care.

Mike

Sr. UX Designer

How has your career grown since starting at the company?

OSH has been a tremendous learning experience for my first full time job. My manager has prioritized building a strong foundation of coding skills, which helped me to learn and transition into our new tech stack. I was given learning resources to develop skills on my own and gained knowledge and best practices from collaborating with teammates.

Molly

Software Engineer

What projects are you most excited about?

I am most excited about our disease progression projects. For these projects, we compile unstructured data and combine it with more traditional health data like labs and vital signs to develop models that give providers the opportunity to proactively manage the health of our patients. At Oak Street, you have a direct impact on the patient's care
