Provide direct case and care management for an assigned patient panel: perform risk assessments, develop individualized care plans, coordinate access and transitions of care, promote self-management, and collaborate with providers and care teams through in-person and telephone contact.

Manage month-end close activities, prepare and post journal entries, reconcile general ledger and liability accounts, maintain chart of accounts and amortization schedules, prepare financial schedules and bank reconciliations, support year-end audits, and assist in preparing internal financial statements and reports.

Lead and oversee the Finance Department including accounts payable, purchasing, and grant accounting. Manage monthly and annual close, prepare audit schedules, support budgeting and financial reporting, ensure regulatory compliance, and recruit/train staff. Provide general ledger oversight for cost reporting, grant billing, and financial controls.
