Provide case management for adults with developmental disabilities: build client/family relationships, coordinate services and referrals, develop care strategies, maintain electronic records (Quantum), advocate during crises, partner with community resources, and perform frequent community visits throughout Cook County.

Coordinate person-centered day services for adults with disabilities: plan activities, advocate for member goals, oversee daily programming, supervise and train staff, assist members accessing benefits and employment, maintain compliance and accurate records to support program success.

Manage and train payroll team to ensure accurate, timely payroll across multiple locations. Maintain compliance with labor, tax, garnishment and benefits rules; audit transactions; reconcile ledgers and prepare reports; handle W-2s, onboarding payroll setup, audits, and employee inquiries; collaborate with Finance and HR and maintain payroll systems (Paylocity/UKG).

Coordinate and oversee individualized programs for members with intellectual, developmental, and psychiatric disabilities. Supervise and train staff, support person-centered planning and implementation, ensure compliance with funding and reporting requirements, maintain member records, maximize community access, respond to medical/behavioral emergencies, assist with benefits/placement, and manage billing and attendance reporting.

Coordinate person-centered day services: plan and oversee center and community activities, advocate for members, supervise and train staff, maintain compliance and records, and help members access benefits, services, and employment resources.

Provide community-based psychiatric nursing for adults with serious mental illness: coordinate medical care and annual evaluations, administer injectable psychotropic medications, monitor labs/vitals/medications, conduct home visits and emergency response, participate in treatment planning and documentation (Medicaid Rule 132/140), coordinate with providers, and provide education and support.
