Coordinate the RAI/MDS process to ensure timely, accurate resident assessments reflecting physical, mental, and psychosocial status. Ensure regulatory compliance, documentation for reimbursement, support care planning with the interdisciplinary team, track schedules, and aid quality improvement and survey readiness.

Provide psychosocial support and advocacy for skilled nursing residents: complete social service assessments, coordinate discharge planning and external psychiatric services, involve families in care planning, connect residents with community resources and facility programs, document plans and collaborate with interdisciplinary care teams.
