The Provider Partnerships Manager will develop relationships in the local healthcare ecosystem, grow referral partnerships, and advocate for older adults’ mental health access.

Guide older adults and their caregivers through the mental healthcare intake process. Responsibilities include providing empathetic support, coordinating care, matching patients with therapists, managing multiple patients through different stages, and communicating clearly during sensitive conversations. The role requires organization, attention to detail, comfort with healthcare and CRM software, and a commitment to improving mental health access for older adults.

Provide evidence-based psychotherapy to older adults through telehealth, develop individualized treatment plans, manage clinical documentation using EMR and AI tools, and participate in case consultations and clinical leadership meetings. The role requires an APA-accredited doctoral degree, an active independent Illinois psychology license, and comfort with virtual care and electronic health records.
