We have an immediate opening for a Clinical Liaison who can build relationships and support care coordination for our community's most vulnerable.

Positive Overview:

The Clinical Liaison is a field-based nurse responsible for coordinating existing Homecare (typically Medicaid) patients that exhibit a change in condition that necessitates a Home-Healthcare (typically Medicare/other) episode. This coordination allows for continuity of care for patients that are Dual Eligible in a facility/short hour and home/long hour situation. The Clinical Liaison works closely with PurposeCare agency Director of Nursing (DON), Director of Care Transition (DCT), Care Transitions Coordinators (CTC), and Marketing to ensure eligible patients receive appropriate credentials for Home-Healthcare, i.e., Doctors Order, Dx, H&P, and F2F.

Position Objectives and Responsibilities

Main tasks and responsibility

  • Coordination of Dual Eligible (DE) patient care and coordination between Agencies.
  • Ensure patient care and caregiver services are aligned to PurposeCare guidelines, local, state and CMS regulations.
  • Develop the coordination with Facilities (ALF, Rehab etc.) and Hospitals to ensure patients are evaluated for Home Healthcare. Ensure that Dr. Order, F2F, Dx and H&P are provided to the Agency for a Home Healthcare episode.
  • Partner with agency Administrator and DON on any surveys or audits from regulating bodies or accrediting agencies.
  • Must have an in-depth working knowledge of current home health care practices/procedures.
  • Must have above average verbal and written communication skills,
  • Must have the ability to think, react quickly and have good judgment in case of emergency,
  • Must have the ability to work independently with little supervision,
  • Must be able to work long hours and maintain flexibility to meet scheduling needs of Agency,
  • Must possess diplomacy and social skills; and enjoy working with geriatrics,

  • Attention to detail: Attention to detail is a skill that can help quality assurance nurses perform their duties effectively. It can also help you identify any issues with the quality of care that you observe during your assessments.
  • Communication skills: Communication is the act of conveying information from one person to another. As a quality assurance nurse, you may be communicating with patients, other medical staff and patients' families. Effective communication can help you to relay information accurately, answer questions and resolve issues.
  • Computer proficiency: Computer proficiency, especially with Electronic Medical Records (EMR) systems is an essential skill that quality assurance nurses.
  • Critical thinking: Critical thinking is the ability to analyze a situation and determine the best course of action. As a quality assurance nurse, you may be required to conduct investigations into the quality of care provided. This may require you to analyze the processes and procedures and determine if they are effective. You may also be required to conduct investigations into patient outcomes and determine if the Clinicians care was the cause. Critical thinking can help you make sound decisions when evaluating a situation.

Required skills and qualifications:

  • Bachelor's degree (BS) in Nursing, or Licensed Practical Nurse with minimum of 5 years of experience
  • Graduate of an accredited school of nursing
  • Current license with state board of nursing
  • 5 years' experience in home health care or related health care field preferred
  • Develops and implements client home health care plans
  • Participates in client case conferences
  • Submits client plans and client clinical record documentation to Case Manager as required
  • Discusses client plans of care with supervising Case Manager
  • Provides clinical home care per policies and procedures of Agency
  • Reports any observations of client conditions to Case Manager
  • Promotes good client relations at all times; assists clients by responding to their requests and meeting their socialization needs
  • Participates in committee meetings
  • Discusses issues with clients and client families in a diplomatic/accurate fashion.
  • Refers clients with financial concerns regarding home health costs to Case Manager
  • Assists in developing cost control policies for needed equipment
  • Notifies Case Manager if equipment needs repair, etc.
  • Records information pertaining to such equipment accurately
  • Assists in promoting an agency environment that is following all federal, state and local laws
  • Maintains confidentiality regarding client
  • Promotes education for home health care staff members
  • Promotes education for clients and client families
  • Participates in education programs and reviews current literature regarding health care
  • Completes all assignments on a timely basis
  • Performs other duties as assigned by Case Manager
  • Ability to travel to patients, facilities (hospital/Rehab etc.) and be on-site at PurposeCare Agency or office.
  • Ability to read, analyze, and interpret clinical-related documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to make effective and persuasive speeches and presentations to employee groups, management at all levels of the company and occasionally to outside organizations.

Qualifications and Requirements:

  • Must hold an LPN or RN license in good standing
  • Positive attitude, great communication skills, and ability to work independently
  • A solid personal character with an empathetic personality
  • Leverage current client relationships and establish new partnerships with physicians, hospitals, clinics and rehab facilities
  • Must be comfortable collaborating with referral sources to generate new business
