Lead and coordinate high-priority transformation initiatives to scale EHR-integrated value-based care insights. Perform rigorous data analysis with BI partners, manage project plans and stakeholder communications, support tool deployment, practice adoption, impact evaluation, and operationalization, and adopt AI tools to increase reach and impact.
Working in the Office of the CEO and reporting to the VP of Point of Care & Risk Stratification, the Transformation Manager will support high-priority transformation initiatives, to aid in Aledade’s performance and growth. This role will initially focus on scaling the delivery and increasing the impact of value-based care insights into partner practice EHR workflows, which is Aledade’s priority transformation initiative in 2026. The team member will support projects spanning analytics, tool deployment, practice adoption, impact evaluation, and operations. The team member will serve as a partner for internal business intelligence, field, and practice engagement teams. Finally, as a member of the Office of the CEO, the hire will have opportunities to contribute to Aledade’s broader transformation efforts, in service of Aledade’s mission to do more good for patients, practices, and society. The team member will be expected to adopt and master a variety of AI tools that help scale their impact and reach.
Primary Duties:
- Research, analyze, report on, and inform Aledade’s EHR workflow efforts, including deployment, adoption, efficacy and impact, and future direction
- Support sophisticated and rigorous data analysis that informs decision making on high priority items in Aledade’s transformation portfolio, partnering with Aledade’s Business Intelligence and Impact Analytics teams
- Build & coordinate project plans, deliverables, and communication across multiple stakeholders for special projects as assigned
Minimum Qualifications:
- Bachelor’s Degree required, preferably in Economics, Business, Healthcare, or a related field
- 5+ years of professional experience or 3+ years of professional experience plus a relevant graduate degree required (MBA, MPH, or MPP), with experience in healthcare strategy / operations consulting strongly preferred
- Familiarity with AI as a business user, using applications beyond chatbots (e.g., Claude Code or Cowork, Codex, etc.)
Preferred Qualifications:
- Master’s Degree (e.g., MBA, MPH, MPP) strongly preferred
- Advanced Excel skills strongly preferred, including the ability to work with large, complex, & often incomplete data sets; conduct one-off complex analyses quickly; and utilize Index / Match, XLOOKUP, FILTER, and other advanced formulas
- Familiarity with value-based care, population health, and primary care
- Strong written and verbal communication skills
- Strong organization capabilities & project management skills
- Commitment to confidentiality and discretion in handling sensitive information
- Interviews will assess fundamental “business analyst” toolkit, including abilities to independently prioritize, solve problems via hypothesis-driven analytical methods, execute with attention to detail, and overcome ambiguity

- Sitting for prolonged periods of time. Extensive use of computers and keyboard. Occasional walking and lifting may be required.
