Saline Care Nursing & Rehabilitation is seeking a compassionate, organized, and motivated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our leadership team as our MDS Coordinator / Nurse Manager.

This leadership position plays a vital role in ensuring quality resident care by coordinating the MDS process, supporting nursing operations, maintaining regulatory compliance, and working closely with the interdisciplinary team to achieve outstanding clinical outcomes.

Schedule
  • Full-Time
  • Monday–Friday
  • Participation in the on-call rotation is required
CompensationRegistered Nurse (RN)
  • $33.00–$35.00/hour
  • +$0.10/hour for each verified year of experience (up to 20 years)
  • +$0.75/hour while serving on-call
  • $15,000 Sign-On Bonus
Licensed Practical Nurse (LPN)
  • $29.00–$31.00/hour
  • +$0.10/hour for each verified year of experience (up to 20 years)
  • +$0.75/hour while serving on-call
  • $10,000 Sign-On Bonus

Sign-on bonuses are paid out over one year.

Responsibilities
  • Coordinate and oversee the MDS assessment and care planning process
  • Complete accurate and timely MDS assessments and submissions
  • Ensure compliance with federal and state regulations
  • Monitor quality measures and support reimbursement accuracy
  • Assist with survey readiness, clinical documentation, and quality improvement initiatives
  • Support nursing staff while promoting exceptional resident-centered care
Qualifications
  • Current Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing
  • Strong organizational, documentation, and critical thinking skills
  • Excellent communication and leadership abilities
  • Long-term care experience preferred
  • Previous MDS experience is preferred but not required—we are willing to train the right candidate
