The Clinical Documentation Associate will be responsible for varying levels of risk adjustment, regulatory compliance and HCC diagnoses. 

How you can make a difference

While this role will be dynamic in nature ultimately the key responsibilities will include:

  • Review, analyze and code diagnostic information in patient charts; identify coding opportunities for improved coding accuracy
  • Validate missed coding opportunities
  • Ensure compliance with established coding guidelines, third party reimbursement policies, regulations and accreditation guidelines
  • Relay findings for review to providers to support clinical documentation accuracy
  • Audit patient charts to ensure accurate coding

This role is designed to allow coding professionals in an apprenticeship/associate status or coders who are inexperienced in clinical documentation and risk adjustment programs to obtain a role to gain valuable coding knowledge and experience.

This role is intended to train those without clinical documentation and risk adjustment experience to learn and eventually progress from the Associate status to a Clinical Documentation Coder. The expectation of this role is that the team member will complete an in-depth educational program to obtain a solid understanding of clinical documentation and risk adjustment processes.

The Clinical Documentation Associates will work through the following during their training period with potential for growth once all milestones are achieved:

  • Successful completion of the required five-week Clinical Documentation University program
  • Achievement of ≥ 95% performance accuracy for four consecutive weeks
  • Achievement of full accuracy and production standards

 Skills for success

  • Professional Coding Certification such as CPC, CCS, CRC; Apprenticeship or Associate status welcomed
  • Recent successful completion of medical coding training course preferred
  • Knowledge of basic medical terminology and anatomy/physiology preferred
  • Familiarity with Electronic Health Records
  • Strong communication skills
  • Technology-savvy (Microsoft Office, Excel, Outlook)
  • High School Diploma required; Bachelor’s degree preferred
