 Position: General Cardiologist 

 

Monogram Health is seeking a full-time General Cardiologist to join our multidisciplinary clinical team. This role is primarily remote, leveraging telehealth to provide cardiovascular expertise within a broader polychronic care model. You’ll collaborate with internal medicine physicians, nurse practitioners, and specialists in cardiology, endocrinology, pulmonology, and behavioral health to support patients managing multiple chronic conditions such as heart failure, diabetes, COPD, and hypertension. 

This is a unique opportunity to practice evidence-based cardiology in a tech-enabled, patient-centered environment focused on proactive care and health equity.


Responsibilities
  • Deliver cardiovascular care to patients with multiple chronic conditions via telemedicine 
  • Apply clinical guidelines to determine appropriate interventions and care pathways 
  • Oversee quality metrics for patient panels, including blood pressure control and medication adherence 
  • Reduce avoidable hospitalizations and emergency visits through targeted care strategies 
  • Supervise Nurse Practitioners and support their clinical development 
  • Collaborate with Chief cardiologist to deliver educational training to NP's 
  • Collaborate with Market Physician Executives on care plans for high-risk or frequently hospitalized patients 
  • Participate in clinical committees and peer-to-peer consultations 
  • Understands of broad range of patient situations 
  • Use data tools to monitor, report, and enhance care quality and utilization 
  • Experience in delivering care to complex patients in a variety of clinical settings 
Position Requirements
  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree  
  • Graduate of an Accredited Cardiology Training Program  
  • Board Certification in Internal Medicine  
  • Board Certified in Cardiology * Required Licensure  
  • Current state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare. Willingness to become licensed in multiple states as company and member needs require.  
  • Experience educating clinicians on cardiac conditions
