Provide clinical case management for up to 20 I/DD clients: develop and implement individualized strategies, coordinate internal/external services, ensure compliance with DHS/HCBS/CARF, maintain electronic records (Cx360), conduct site visits, oversee behavioral and health services, manage client finances, provide direct support/transportation, and train/monitor staff.

Manage day-to-day operations of a 24/7 Crisis Response Program (Crisis Line and Mobile Crisis Response). Recruit, hire, train, schedule and supervise staff/volunteers; provide on-call supervision and crisis intervention as needed; ensure compliance, documentation, billing, Cx360 recordkeeping, program development, stakeholder relations, and community referrals. Must be available for evenings, overnights, weekends, and rotating on-call shifts.

Provide mobile crisis intervention and community-based support services for individuals with mental illness. Perform assessments, crisis counseling, treatment planning, documentation per Rule 132, and coordinate care with providers. Work primarily off-site, maintain certifications, meet productivity standards, and participate in team supervision and meetings.
