Leads technical onboarding and integration of clinical and claims data for healthcare practices, payers, and vendors. Manages EMR, API, SFTP, and third-party integrations; translates clinical requirements into technical workflows; creates documentation and data-flow diagrams; performs data QA and validation; troubleshoots issues; coordinates stakeholders; and manages go-live planning, deployment, and post-implementation support.

Engages patients remotely and in person to provide nonclinical care navigation, peer support, education, referrals, and community resource connections. Assesses medical, behavioral, social, and financial needs; coordinates follow-up; supports care-gap closure, quality measures, risk adjustment, and value-based care goals. Documents interactions in EHR systems, escalates concerns to clinical teams, and collaborates with providers, caregivers, and community organizations while maintaining HIPAA compliance.

Remote coordinator responsible for managing quality improvement (QI) measures and patient outreach to close care gaps. Collects and abstracts data from EMRs, claims, and provider outreach; manages multiple systems, ensures NCQA/HEDIS adherence, prioritizes reporting deadlines, communicates performance and improvement strategies, and collaborates with payers, providers, and internal teams.
