Homethrive

Homethrive is a healthtech startup with a scalable national solution meeting the needs of aging seniors as well as their caregivers. We help seniors age in place while reducing the stress of caregivers by offering multiple solutions tailored to family needs. We assign a dedicated social worker (CareGuide) to work with each family; the family will work with the same CareGuide throughout the program. CareGuides form a trusted relationship and provide varying levels of impactful services as needed. These range from counsel to employees about their aging loved ones and managing respective conditions to a fulsome offering involving assessments of the seniors’ cognitive, physical, and social wellbeing (including recommendations on home safety for prevention) to ongoing coaching of the senior and family to drive behavior change such as improved medication adherence. We also recommend and coordinate third party services including medication, meal and grocery delivery, home care, transportation and social activities.
