Teams at TaskForce Prevention and Community Services

Provides client-centered case management for people living with HIV, including assessments, individualized care planning, referrals, care coordination, advocacy, outreach, retention support, documentation, and data entry. Collaborates with medical and community service providers, participates in multidisciplinary meetings, maintains confidentiality, and supports clients in accessing HIV care, behavioral health, housing, substance use, and other services.
