Description of Work

This position is 100% Federally Funded and Time Limited for the duration of the RHTP funding. This position serves as the primary programmatic leader and day-to-day partner to ROOTS Hub Leads within their assigned region(s). This position functions as the central connector between regional implementation and the statewide NC ROOTS strategy, ensuring that hubs receive aligned, timely, and coordinated technical assistance across all program initiatives. This position assesses regional needs, supports strategic planning, and provides ongoing guidance related to policy, governance, and operational readiness. A core responsibility of this position is to coordinate specialized technical assistance across Initiatives 1, 2, 4, and 5—facilitating access to subject matter experts in primary care and chronic disease, workforce development, and financial sustainability. In addition, this position plays an essential bridging role to the NCDHHS Grants Management & Operations Team, ensuring that hub program activities are implemented with quality and in alignment with budget requirements, procurement processes, program monitoring expectations, and state and federal regulations. Through this integrated support model, this position strengthens hub capacity, promotes consistency across regions, and advances the overarching goals of the Regional Health Transformation Program (RHTP).

Salary Grade Range: $67,122.00 - $ 117,465.00  

Recruitment Range: $67,122.00.00 - $88,855.00

About the Rural Health Transformation Program

NC Rural Health Transformation Program is a statewide program led by NCDHHS to transform rural health care delivery, improve health outcomes, and ensure sustainable access to care for nearly 3 million rural North Carolinians. The program is funded over five years by the Centers for Medicare & Medicaid Services (CMS).

The main goals of the program are to: 

  • Catalyze innovative health care models for rural communities by changing the way providers work together to care for patients in rural NC. 

  • Transform the rural care experience, building community-based clinical, behavioral, and social supports close to home. 

  • Create a sustainable rural health delivery system through underlying systems changes in rural workforce pipelines and care team models, and rural provider financial models.
