The Lutheran Home Skilled Nursing Community is hiring Social Services Coordinators to support resident well-being and coordinate psychosocial services for our residents and their families.
What You'll Do
- Complete social service assessments to identify each resident's needs and preferences
- Coordinate discharge planning and connect residents with outside psychiatric and psychosocial services
- Advocate for residents and support families through care transitions
- Facilitate family involvement in care planning meetings
- Connect residents with community resources and encourage participation in facility programming
- Document social service plans and progress notes in PointClickCare
- Collaborate with nursing, therapy, and the rest of the care team on resident care plans
What You'll Need
Required:
- Minimum 2 years of experience in a Social Services role within a senior living setting
- 2+ years of discharge planning experience in senior living
- 1+ year of experience using PointClickCare (PCC)
Preferred:
- Bachelor's degree in social work, human services, or a related field
- Experience working with Medicare/Medicaid resident populations
