The Lutheran Home Skilled Nursing Community is hiring Social Services Coordinators to support resident well-being and coordinate psychosocial services for our residents and their families.

What You'll Do

  • Complete social service assessments to identify each resident's needs and preferences
  • Coordinate discharge planning and connect residents with outside psychiatric and psychosocial services
  • Advocate for residents and support families through care transitions
  • Facilitate family involvement in care planning meetings
  • Connect residents with community resources and encourage participation in facility programming
  • Document social service plans and progress notes in PointClickCare
  • Collaborate with nursing, therapy, and the rest of the care team on resident care plans

What You'll Need

Required:

  • Minimum 2 years of experience in a Social Services role within a senior living setting
  • 2+ years of discharge planning experience in senior living
  • 1+ year of experience using PointClickCare (PCC)

Preferred:

  • Bachelor's degree in social work, human services, or a related field
  • Experience working with Medicare/Medicaid resident populations
