Position: Director, Project Management

Report to: Chief Growth Officer

Why work at Concerto? What is in it for you?

This is a unique opportunity to build an enterprise Project Management function from the ground up while remaining close to the work. You will shape how Concerto plans, executes, tracks, and scales complex growth initiatives, partner directly with senior leaders across the organization, and have the opportunity to build a team as the function and project portfolio grow.

Who Are We?

Concerto Renal Services is an industry leader in sub-acute hemodialysis within a skilled nursing facility and/or long-term care facility. We are a rapidly expanding business with a simple mission: reimagining dialysis care, one community at a time. Our model focuses on ensuring those with end-stage renal disease receive the best possible care through a more convenient, centralized, continuous care model.

Why is this Role Essential?

Concerto is growing rapidly across multiple markets, and our continued expansion requires a more disciplined, scalable approach to enterprise project execution. The Director, Project Management will establish and lead Concerto’s Project Management function, creating the structure, systems, governance, and accountability needed to move complex initiatives forward. This is not a traditional PMO leadership role with an established team and infrastructure. The Director will be both a function builder and a hands-on project leader - developing the model while personally driving critical projects and ensuring execution across the organization.

What Will You Do?

  • Build and lead Concerto’s enterprise Project Management function, including the operating model, governance, methodology, tools, standards, and future team structure.
  • Personally lead a high-volume portfolio of complex, cross-functional projects, initially including new clinic construction and openings, new-state entry, and unit takeovers.
  • Create scalable project-management infrastructure, including SOPs, POCs, stage gates, templates, master project plans, meeting cadences, dashboards, escalation pathways, and executive reporting.
  • Develop and maintain project plans with clear milestones, owners, dependencies, deadlines, risks, decisions, readiness requirements, and transition criteria.
  • Drive execution across Design/Construction, Facility & Water Services, Operations, Clinical, Compliance, Talent Acquisition, Logistics, IT, and other functions while maintaining clear subject-matter ownership.
  • Lead project meetings and working sessions, document commitments, follow up relentlessly, resolve ambiguity, and escalate barriers when progress stalls.
  • Establish portfolio-level visibility so leadership has accurate, current insight into project status, timing, risks, dependencies, ownership, and next steps.
  • Develop repeatable playbooks for clinic openings, new-state entry, takeovers, and other recurring enterprise initiatives.
  • Partner with internal and external stakeholders on project scopes and client-facing quotes to ensure work is appropriately defined, complete, and aligned before execution.
  • Communicate effectively with SNF partners, contractors, vendors, and other external stakeholders throughout project development and implementation.
  • Establish clear readiness gates and structured handoffs to Operations and other functional owners.
  • Identify lessons learned and continuously improve project standards, workflows, and organizational execution.
  • Define the future PM organizational structure and, as business needs support growth, recruit, develop, and lead a high-performing Project Management team.
  • Expand the PM function over time to support additional enterprise initiatives such as technology implementations, operational rollouts, site transitions, and other strategic projects designated by leadership.

Compensation: $120,000-$150,000 annually,

*Salary is commensurate with experience and qualifications. Eligible for additional compensation and benefits in accordance with company programs.

Qualifications

You Might Be a Fit If You are…

  • A builder who enjoys creating structure where it does not yet exist and can translate ambiguity into a practical operating model.
  • A highly experienced project-management leader who is equally comfortable setting strategy and rolling up your sleeves to build a project plan, run a meeting, update a dashboard, or chase a critical dependency.
  • Comfortable leading through influence and holding senior, cross-functional stakeholders accountable without relying on direct authority.
  • Highly organized, decisive, and persistent, with exceptional follow-through across many simultaneous workstreams.
  • Able to see the enterprise picture while remaining close enough to the details to identify risk early and keep projects moving.
  • A clear, concise communicator who can move easily between executive-level reporting and detailed working sessions.
  • Comfortable operating in a rapidly growing organization where processes are evolving and the answer is not always already defined.
  • A leader who can build credibility quickly and develop a scalable team and function as organizational needs grow.

What’s Required?

  • 8+ years of progressive project, program, implementation, operations, construction, or related leadership experience, including significant responsibility for complex, cross-functional initiatives.
  • Demonstrated experience building, formalizing, or materially improving a project-management function, PMO, governance model, or enterprise project-management infrastructure.
  • Proven ability to personally lead multiple complex projects from initiation through implementation and transition while simultaneously improving the systems used to manage the portfolio.
  • Experience operating with senior executives and influencing leaders across multiple functions.
  • Experience developing project governance, portfolio reporting, risk/escalation processes, stage gates, and standardized project-management tools.
  • Strong business judgment and the ability to distinguish project/process ownership from functional and technical ownership.
  • Working knowledge of construction, implementation, or multi-site operational sequencing strongly preferred; deep technical construction or dialysis-water expertise is not required.
  • Experience with modern project-management platforms and dashboards; Monday.com or similar tools preferred.
  • Bachelor’s degree in Business, Healthcare Administration, Project Management, Operations, or a related field preferred, or equivalent relevant experience.
  • PMP, PgMP, Lean Six Sigma, or comparable project/program management certification is preferred but not required.
