Stonebridge Nursing & Rehabilitation is seeking an organized, detail-oriented, and motivated RN or LPN to join our leadership team as our MDS Coordinator.

This position plays an important role in resident care, clinical documentation, reimbursement accuracy, regulatory compliance, and quality outcomes. We're looking for a nurse who is ready to use their clinical knowledge in a specialized role while working closely with nursing leadership and the interdisciplinary care team.

CompensationRegistered Nurse (RN)

$33.00–$35.00/hour

Licensed Practical Nurse (LPN)

$29.00–$31.00/hour

Additional Pay
  • +$0.75/hour for on-call requirements

  • Annual performance evaluation with the potential for a pay increase based on performance

What You'll Do
  • Coordinate and oversee the MDS assessment process

  • Complete accurate and timely MDS assessments and submissions

  • Coordinate resident care planning with the interdisciplinary team

  • Monitor clinical documentation for accuracy and completeness

  • Support reimbursement accuracy and regulatory compliance

  • Monitor quality measures and resident outcomes

  • Assist with survey readiness and quality improvement initiatives

  • Communicate with nursing staff regarding documentation and resident care needs

  • Work closely with facility and nursing leadership

What We're Looking For
  • Current Illinois RN or LPN license in good standing

  • Strong clinical assessment and documentation skills

  • Excellent organizational and time-management abilities

  • Strong communication and critical-thinking skills

  • Ability to collaborate effectively with the interdisciplinary team

  • Long-term care or skilled nursing experience preferred

  • Previous MDS experience preferred
