Carrier Mills Nursing & Rehabilitation Center is seeking an experienced MDS Nurse Manager (RN or LPN) to join our leadership team.

This is a great opportunity for an MDS professional looking for a Monday–Friday schedule, competitive pay, a $10,000 sign-on bonus, and the opportunity to play an important role in resident outcomes and facility success.

Schedule
  • Full-Time

  • Monday–Friday

  • 8:00 AM – 4:30 PM

  • Participation in the management on-call rotation is required

Compensation

Registered Nurse (RN): $33.00–$35.00/hour

Licensed Practical Nurse (LPN): $29.00–$31.00/hour

  • $10,000 Sign-On Bonus

  • Sign-on bonus paid over one year

  • Annual performance evaluation with the potential for a pay increase

What You'll Do

As our MDS Nurse Manager, you'll oversee and coordinate the MDS process while working closely with nursing leadership and the interdisciplinary team to promote accurate documentation, regulatory compliance, reimbursement accuracy, and excellent resident outcomes.

Key responsibilities include:

  • Coordinating and completing MDS assessments

  • Leading and supporting interdisciplinary care planning

  • Monitoring documentation, case mix, and reimbursement

  • Supporting quality measures and survey readiness

  • Maintaining compliance with CMS and regulatory requirements

  • Collaborating with clinical leadership to improve resident outcomes

What We're Looking For
  • Current Illinois RN or LPN license

  • Previous MDS and care planning experience

  • Long-term care experience preferred

  • Knowledge of Medicare, Medicaid, and reimbursement systems preferred

  • Strong organizational and communication skills

  • Excellent attention to detail

  • Ability to collaborate effectively with an interdisciplinary team
