Saline Care Nursing & Rehabilitation is looking for an organized and motivated RN or LPN to join our leadership team as an MDS Coordinator / Nurse Manager.

Already have MDS experience? We'd love to put that experience to work.

Full-Time | Monday–Friday

Participation in the management on-call rotation is required.

$33.00–$35.00/hour

• +$0.10/hour for each verified year of experience, up to 20 years

• +$0.75/hour while serving on-call

• $15,000 Sign-On Bonus

$29.00–$31.00/hour

• +$0.10/hour for each verified year of experience, up to 20 years

• +$0.75/hour while serving on-call

• $10,000 Sign-On Bonus

Sign-on bonuses are paid over one year.

Plus, an annual performance evaluation with the opportunity for a merit-based pay increase.

As our MDS Coordinator / Nurse Manager, you'll play an important role in resident care, clinical quality, and nursing operations.

• Coordinate the MDS assessment and care planning process

• Complete accurate and timely MDS assessments and submissions

• Support reimbursement accuracy and regulatory compliance

• Monitor quality measures and resident outcomes

• Assist with survey readiness and clinical documentation

• Support quality improvement initiatives

• Collaborate with nursing leadership and the interdisciplinary team

• Provide support and guidance to nursing staff

• Help maintain high standards of resident-centered care

• Current Illinois RN or LPN license in good standing

• Strong organizational and documentation skills

• Excellent communication and critical-thinking abilities

• Interest in nursing leadership and clinical quality

• Long-term care experience preferred

• Previous MDS experience preferred, but not required

• We are willing to train the right candidate
