You'll make sure every resident's assessment accurately reflects their real physical, mental, and psychosocial status, work that directly determines the care plan they receive and how the facility is reimbursed for that care.

What you'll do:

  • Coordinate the RAI process so resident assessments are completed accurately and on time
  • Make sure MDS assessments reflect each resident's physical, mental, and psychosocial status
  • Keep the facility in compliance with Medicare, Medicaid, OBRA, and other payer requirements
  • Verify documentation supports the services provided and reimbursement accuracy
  • Work with the interdisciplinary team on care planning, assessment completion, and case management
  • Track assessment schedules to make sure regulatory timelines are met
  • Support quality improvement work and survey readiness

What you'll need:

Required:

  • Graduate of an accredited school of nursing, with a current RN license in Illinois
  • At least 2 years of MDS 3.0 and case management experience in a skilled nursing or post-acute setting
  • Comfortable working independently while coordinating with an interdisciplinary team
  • Working knowledge of Microsoft Office

Preferred:

  • Long-term care or skilled nursing experience
  • Current RAC-CT certification, or willingness to obtain it within 6 months of hire and keep it current

Pay and benefits:

  • $36.50–$49.00/hour, based on experience
